About the Role

We are seeking a highly skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Home Health Nursing Care Specialist. In this role, you will be responsible for providing quality nursing care to patients with complex medical needs.

Responsibilities

  • Provide skilled nursing care to patients in their homes
  • Evaluate patient needs and develop individualized care plans
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Conduct ongoing assessments to monitor patient progress and adjust care plans as needed

Requirements

To be successful in this role, you must have:

  • A valid RN or LPN license in the state of Indiana
  • Experience working with medically fragile patients
  • Strong communication and interpersonal skills
  • The ability to work independently and as part of a team
