Job Summary

Southwest Behavioral & Health Services is seeking a dedicated and empathetic Intake Specialist to join our dynamic Flagstaff team. As a key member of our Pathways Program, you will play a vital role in helping individuals transition from incarceration to community stability.

Key Responsibilities
  • Complete intake and enrollment for individuals released from incarceration
  • Facilitate connections to outpatient services, behavioral health, and psychiatric medication appointments as needed
  • Provide crisis triage support and linkages to care for clients accessing services
  • Work collaboratively with a multi-disciplinary team to ensure seamless transitions
  • Develop and implement tracking systems for referrals and follow-up processes
Requirements
  • High School Diploma or GED plus some college coursework in Behavioral Health or related fields
  • One year of directly related job experience or equivalent combination of experience and education
  • Valid Arizona driver's license and ability to transport clients as needed
  • Fingerprint clearance issued by the Arizona Department of Public Safety or eligibility to receive within eight weeks of hire
