Physical Therapist

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers. As a Physical Therapist with our organization, you will have the opportunity to make a meaningful difference in the lives of our patients while enjoying a positive and supportive work environment.
Key Responsibilities:
- Conduct comprehensive assessments of patients' needs, including OASIS assessments, to develop and implement effective treatment plans.
- Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and patient outcomes.
- Teach, supervise, and counsel patients and their families on physical therapy programs, promoting independence and empowerment.
- Communicate effectively with patients, families, and healthcare teams to ensure timely and accurate information exchange.
- Supervise Physical Therapist Assistants and provide guidance and support as needed.
Requirements:
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- At least one year of experience as a Physical Therapist, with home healthcare experience preferred.
