Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Clinical Triage Nurse. This role is responsible for providing expert clinical guidance and support to patients via the contact center, ensuring they receive the right level of care at the right time.

Key Responsibilities
  • Provide telephonic triage services for patients contacting the contact center, utilizing clinical triage processes, protocols, and procedures to effectively navigate patients to the right site and time of care.
  • Understand patient needs and offer recommendations, education, or care advice to triage patients and schedule them with the appropriate level of care.
  • Participate in care coordination for patients, working collaboratively to best understand their needs and aiming to reduce readmissions, enhance chronic disease management, and manage any health risks for the patient.
  • Assist patients in scheduling provider appointments and communicate with providers as necessary.
  • Manage work to KPIs, mission, values, and objectives of the Clinical Triage work center, ensuring accurate and timely documentation in the EMR according to best practice guidelines.
  • Participate in virtual monitoring, subsequent reporting, and escalation to support services, providing additional support to ensure patient needs are met.
Requirements
  • Bachelor's of Science Degree in Nursing
  • 2+ years of related work experience as an RN
  • 1+ years of experience in clinical practice is preferred
  • 2+ years of experience in acute care, critical care, or emergency department work is preferred
  • 1+ years of experience with telephone triage is preferred
  • 1+ years of experience with Epic is preferred
  • 1+ years of experience in using electronic triage software is preferred
Preferred Qualifications
  • Bilingual (English/Spanish) preferred
  • Ability to understand patient needs and efficiently guide them to necessary level of care
  • Ability to work with clinical technology to fulfill process and workflow needs
  • Ability to multi-task, prioritize in a fast-paced environment and meet deadlines with minimal supervision
  • Excellent communication skills both written and verbal
Working Conditions
  • Ability to work in surroundings with degree of dust, noise, odors, etc. without undue discomfort
  • Ability to hear, talk, walk, stand, bend and stretch
  • Ability to read, write and interpret documents
  • Ability to sit for long periods of time at desk to perform job functions
  • Ability to communicate effectively with clients and co-workers
