Responsibilities:

  • Provide direct patient care and implement care plans
  • Document patient information accurately and timely
  • Assess and educate patients and their families on self-care techniques
  • Assist RNs and physicians with specialized procedures and duties
  • Participate in coordination of home health services and report identified needs to the RN and Clinical Supervisor

Requirements:

  • Eligibility to work in the United States
  • CPR Certification and PPD Certification
  • At least 1 year of experience as an LPN
  • Valid LPN License with at least 1 year of work experience
  • Valid Driver's license
  • Ability to work evening and weekend shifts
  • Reliable transportation
