We are seeking an experienced LPN Supervisor to join our team at Brookridge A Thrive More Community in Winston Salem, NC. As an LPN Supervisor, you will play a vital role in providing exceptional care to our residents and upholding our core values of compassion, friendliness, and excellent communication skills.

Responsibilities:
  • Visit patients in their homes for one-on-one visits to assess their physical, psychosocial, and environmental needs.
  • Complete initial and ongoing assessments to identify the needs of home health patients and chart observations.
  • Assess patients' conditions, perform evaluation tasks, and administer medication as prescribed by the patient's physician.
  • Educate patients and their families on proper home health care strategies and procedures.
  • Provide effective communication to patients/clients, their family members, team members, and other health care professionals.
Qualifications:
  • Current state license as an LPN.
  • Proof of current CPR certification.
  • Compassionate, friendly demeanor.
  • Excellent communication skills.
  • Current driver's license and reliable transportation.
  • Ability to work a flexible schedule and travel locally.
