Job Summary:
We are seeking a skilled Physical Therapist to join our team at Moraga Post Acute. As a Physical Therapist, you will play a vital role in helping our patients achieve their rehabilitation goals.

Key Responsibilities:
Develop and implement effective treatment plans in collaboration with our interdisciplinary team.
Treat patients with a range of conditions, from acute injuries to chronic conditions, using evidence-based practices.
Work closely with our nursing staff to provide training and support to Restorative Aides.
Supervise Physical Therapy Assistants in direct patient care and related activities, ensuring compliance with state practice acts.
Communicate effectively with colleagues and supervisors regarding patient progress, challenges, and plans.
Participate in patient care conferences, utilization review meetings, and rehabilitation conferences as needed.
Provide in-services training to other staff members on physical therapy techniques and best practices.
Record and maintain accurate patient evaluations, treatment notes, and progress reports, adhering to PT Board state practice acts and regulatory requirements.
Ensure seamless transitions for patients by instructing families or nursing staff on maintenance programs and caregiver training.
Participate in discharge planning to ensure patients receive necessary support and resources.
Recommend and facilitate the ordering of durable medical equipment to enhance patient mobility and independence.
Report any equipment issues to ensure timely maintenance and repair.
Develop strong relationships with patients, families, staff, and professional colleagues through effective communication and collaboration.

Requirements:
A valid Physical Therapist license is required for this position.
