Position Overview:
The Clinical Pharmacist I plays a crucial role in the healthcare team by ensuring optimal medication management for patients. This position involves the review and monitoring of patient treatment plans, ensuring they align with the latest clinical and institutional standards.
Key Responsibilities:
- Evaluate and oversee patient therapy plans for suitability based on established clinical guidelines and individual patient needs.
- Identify and address medication-related issues and potential misuse.
- Review all medication orders to confirm compliance with the standards and practice guidelines of the organization.
- Provide expert advice to healthcare providers regarding drug selection, taking into account laboratory results and the patient's clinical status.
- Ensure adherence to safety protocols in the preparation and handling of chemotherapy and hazardous materials, following current guidelines.
- Engage in educational initiatives and respond to inquiries related to drug information.
- Contribute to the training and mentorship of pharmacy staff, including students and residents.
- Participate in quality improvement and safety projects related to oncology services.
Qualifications:
- Graduation from an accredited pharmacy school.
- Active pharmacy license in the relevant state.
- Minimum of one year of experience in a specialized area or completion of a PGY-1 Pharmacy Residency.
- Alternatively, completion of a PGY-2 Pharmacy Residency in a specialized field.
