Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as a RN Case Manager. In this role, you will be responsible for delivering high-quality patient care, collaborating with our interdisciplinary team, and ensuring that our patients receive the best possible support during their end-of-life journey.

Responsibilities
  • Conduct initial nursing evaluations to determine patient eligibility for hospice services
  • Develop and implement individualized plans of care to meet the unique needs of each patient
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner
  • Participate in ongoing education and training to stay up-to-date on best practices in hospice care
Requirements
  • Current RN licensure in the state of practice
  • One year of clinical experience in a healthcare setting
  • Current CPR certification
  • Valid driver's license and reliable transportation
