Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our Hospice team as a RN Case Manager. In this role, you will be responsible for delivering high-quality patient care, collaborating with our interdisciplinary team, and ensuring that our patients receive the best possible support during their end-of-life journey.
Responsibilities- Conduct initial nursing evaluations to determine patient eligibility for hospice services
- Develop and implement individualized plans of care to meet the unique needs of each patient
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
- Document patient progress and outcomes in a timely and accurate manner
- Participate in ongoing education and training to stay up-to-date on best practices in hospice care
- Current RN licensure in the state of practice
- One year of clinical experience in a healthcare setting
- Current CPR certification
- Valid driver's license and reliable transportation
