Job Summary

As a Home Health RN with TriStar Healthcare at Home, you will provide coordinated skilled nursing care to patients of all age groups in the comfort of their own homes. You will work collaboratively with the interdisciplinary team to establish and achieve patient goals, ensuring high-quality patient care. This role requires strong assessment and communication skills, as well as the ability to work independently and as part of a team.

Responsibilities
  • Assess patients' physical, psychosocial, and environmental needs, identifying areas for improvement and developing individualized care plans.
  • Complete OASIS assessments and visit paperwork accurately and efficiently, ensuring timely communication with the interdisciplinary team and patients' families.
  • Communicate significant findings, problems, and changes to the Clinical Manager and physician, documenting all interactions and interventions.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs, ensuring quality and continuity of services.
  • Participate in on-call rotation and emergency call, providing 24/7 support to patients and families.
Requirements
  • Graduate of an accredited school of professional nursing.
  • Minimum of one or more years of home health, public nursing, or acute hospital nursing experience.
  • Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS.
  • Reliable transportation and proof of valid automobile liability insurance.
  • Current BCLS Certification prior to providing patient care.
