**Job Summary:**
We are seeking a skilled Licensed Practical Nurse to join our team at Equiliem Healthcare. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and support to our residents in a long-term care setting.
**Responsibilities:**
* Administer medications and treatments as assigned by the facility under the direction of the managing team, Director of Nurses and/or Physician.
* Observe residents and report changes in their condition to the healthcare team.
* Take and record vital signs, including temperature, pulse, respiration, and blood pressure.
* Obtain specimens for analysis and prepare and apply simple dressings as needed.
* Provide personal care to residents, including bathing, feeding, and making beds.
* Maintain a clean and safe environment for residents and staff.
* Participate in the development and enforcement of policies and procedures.
* Collaborate with the interdisciplinary team to coordinate resident care plans.
* Supervise subordinate staff as needed.
**Requirements:**
* Currently a Licensed Practical Nurse in New York State.
* Preferred long-term care/skilled nursing experience.
**Work Schedule:**
* Evenings (with occasional days/nights).
* Part-time (24 hours a week), every other weekend, and some holidays.
* Per diem - 1-8 hour shift a week, with 1 weekend every month.
