We are seeking a compassionate and skilled Hospice Registered Nurse Case Manager to join our team at Interim HealthCare Hospice - Sacramento, CA. As a key member of our interdisciplinary care team, you will provide high-quality, patient-centered care to individuals with serious illnesses, focusing on comfort, dignity, and quality of life.
Key Responsibilities- Conduct Comprehensive Assessments: Evaluate patients' physical, emotional, psychosocial, and spiritual needs to develop individualized care plans.
- Coordinate Interdisciplinary Care: Collaborate with healthcare professionals, including physicians, social workers, chaplains, and volunteers, to ensure seamless care delivery.
- Education and Support: Empower patients and their families with knowledge about disease progression, symptom management, medication management, and end-of-life care options.
- Documentation and Quality Improvement: Maintain accurate records of patient assessments, care plans, interventions, and outcomes, and participate in quality improvement initiatives to enhance care quality.
- Compassionate End-of-Life Care: Provide symptom management, comfort measures, and support to patients and their families during the dying process.
- Nursing Experience: Minimum of 1 year of nursing experience in hospice care.
- Clinical Proficiency: Strong clinical skills and knowledge of state and federal home health regulations.
- Education and Certification: Graduate of an accredited nursing program and active RN license in California, with CPR certification.
