Palliative Care Specialist: We are seeking a compassionate and skilled Palliative Care Specialist to join our team. In this role, you will work closely with patients, families and healthcare providers to assess and identify needs, develop individualized plans of care, and provide coordinated support services.

Your Key Responsibilities:
* Assess and identify patient/family needs, utilizing the nursing process
* Develop and implement individualized plans of care that accurately reflect patient's evolving needs
* Provide education and support to patients, families and caregivers on disease processes, decline, palliative interventions, caregiving and safety practices
* Participate in interdisciplinary group (IDG) meetings to evaluate referrals/admissions for level of care appropriateness
* Communicate effectively with physicians, staff members, patients, families and supervisors using positive approaches

Requirements:
* Current licensure as Registered Nurse (RN) in the state where employed
* Minimum one year of in-patient nursing or hospice experience

* Ability to use equipment with visual and auditory mechanisms
* Ability to effectively communicate in English (verbal and written)

* Certification in hospice and palliative care
