POSITION SUMMARY
The Registered Nurse plays a vital role in delivering nursing care with a focus on community health education and outreach. This professional is responsible for the planning, organization, and management of home care services, utilizing community resources to tailor services that address the unique needs of individuals and families.

REQUIREMENTS
- Current and unencumbered Registered Nurse (RN) license in the state of employment.
- Bachelor's degree in Nursing from an accredited institution is preferred.
- One to two years of recent acute care experience in a healthcare setting.
- Preferred experience in home health care for at least one year.
- Direct supervisory experience of Licensed Vocational Nurses and Nurse Aides/Assistants is a plus.
- Valid CPR certification required, obtained through in-person training with practical skills evaluation.
- Familiarity with OASIS documentation is preferred.
- Strong observational, verbal, and written communication skills, along with problem-solving abilities and basic math skills.
- Access to reliable transportation is necessary.
