Key Responsibilities- Evaluate patients based on physician referral/orders and develop effective treatment plans with functional, goal-based outcomes
- Treat patients according to treatment plans and record clinical documentation according to accepted regulatory and professional guidelines
- Participate in interdisciplinary team meetings, patient care conferences, utilization review meetings, and other meetings as required or directed
- Instruct families and nursing staff regarding restorative/home programs and participate in discharge planning
To be successful in this role, you must demonstrate a positive attitude and team-building approach with all patients, clients, and team members. You should consistently attend work and provide appropriate notice for scheduled paid time off, maintain productivity standards, and demonstrate knowledge and application of Medicare and other reimbursement regulations.
