We are seeking a dedicated and compassionate Nurse Practitioner to join our team at Assured Hospice of Portland, a part of LHC Group. This is a PRN position that offers the opportunity to make a meaningful difference in the lives of patients and their families.

In this role, you will provide consultation in palliative care, symptom management, and supportive care to meet the needs of our Hospice patients as requested by the referring physician. You will also make face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and report clinical findings to the Hospice Medical Director.

Key Responsibilities:
  • Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director.
  • Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed.
  • Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care.

The estimated salary for this position is $120,000 - $150,000 per year, depending on experience.
