Job Title: Virtual Care Center Registered Nurse

We are seeking a skilled and compassionate Registered Nurse to join our Virtual Care Center team. As a Registered Nurse, you will provide high-quality patient care in the comfort of patients' homes, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical and emotional needs, developing personalized care plans to address their unique requirements.
  • Deliver evidence-based care, adhering to established protocols and guidelines to ensure optimal patient outcomes.
  • Collaborate with patients, families, and healthcare professionals to coordinate care, address concerns, and promote patient engagement.
  • Document patient progress, identifying areas for improvement and implementing changes to enhance care delivery.
Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
