We are seeking a dedicated and skilled Physical Therapist to join our dynamic healthcare team at Summit Healthcare Solutions.
Job Summary:The successful candidate will be responsible for assessing, planning, and implementing rehabilitation care plans to help patients recover from injuries, illnesses, and surgeries.
Key Responsibilities:- Perform initial and ongoing physical therapy evaluations according to American Physical Therapy Association (APTA) standards of practice and Missouri Department of Health standards.
- Develop personalized treatment plans to help patients restore function, alleviate pain, and improve mobility.
- Conduct a comprehensive assessment to identify impairments, functional limitations, and contributing factors to develop a holistic understanding of patient's condition.
- Utilize a variety of interventions, including therapeutic exercises, manual therapy techniques, modalities, and patient education, to address impairments, improve function, and promote overall well-being.
- Promote therapeutic exercises, functional activities, and neuromuscular re-education exercises to enhance strength, flexibility, and motor control.
- Develop individualized care plans based on assessment findings for treatment modalities to patient goals, closely monitoring their responses and adjusting treatment parameters as necessary using evidence-based practice guidelines.
- Guide patients through functional mobility training, gait training, and Activities of Daily Living (ADL) retraining to improve their ability to perform essential tasks and participate in meaningful activities.
- Incorporate real-life scenarios and environmental modifications to facilitate the transfer of skills to home and community settings.
- Implement a plan of treatment on a consistent schedule and adjust this schedule and plan according to patient progress and requirements.
- Evaluate mobility, strength, and current level of patient function and develop goals which are realistic, attainable, and are scaled to patient functional abilities.
- Monitor patients condition and document observations accurately.
