Job Summary

Care Resource is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in a telehealth setting. As a key member of our Patient Care Team, you will provide direct patient care based on nursing assessment and provider orders/protocol, ensuring the health, comfort, and safety of our patients.

Responsibilities
  • Provide pre-visit planning and intake for patients with diverse problems and medical histories.
  • Administer nursing care under the supervision of a registered nurse, ensuring accurate charges for all procedures/tests.
  • Support the Medical Department by improving quality-of-care delivery, reducing cost per visit, and enhancing the patient experience.
  • Assist with the management and maintenance of department supplies/materials/equipment and logs as needed.
  • Notify supervisor when medical equipment needs repair or replacement.
  • Train support staff (Patient Care Assistant, Laboratory Assistants, Receptionist/Medical Clerk) to ensure high-quality and safe care.
  • Actively promote patient-centered care by maintaining effective communication with patients regarding wait time, patient needs, and expectations.
  • Complete tasks sent via EHR by the requested timeframe.
  • Ensure compliance with health center policies and procedures.
  • Provide nursing triage under the supervision of the RN if needed.
  • Participate in meetings, audits, site visits, and support other communication efforts with external contacts as required.
  • Ensure materials/supplies/equipment are fully stocked and available.
  • Assist Medical Providers as requested.
  • Perform other duties as directed, including on-site clinic assignments.
Requirements
  • High school diploma required.
  • Licensed Practical Nurse (LPN) required. BLS-Basic Life Saving for Healthcare Workers, Biomedical Waste, OSHA certifications are required.
  • ACLS-Advance Cardiac Life Saving certification is preferred.
  • Bilingual (English Spanish or English-Haitian Creole) is preferred.
  • Ability to problem-solve and pay attention to detail.
  • Excellent communication skills - oral and written.
  • Computer knowledge (i.e. Microsoft, Electronic Health Records).
  • Organizational, multitasking, and teamwork skills are required.
  • Ability to work with multicultural and diverse populations.
Physical Requirements

This work requires constant talking in person, talking on the phone, hearing/visual acuity, and exposure to blood-borne pathogens and hazardous materials. Frequent walking, standing, sitting, and bending are also required.
