Nurse (LPN or RN) - Part Time/Per-Diem Position

We are seeking a skilled and compassionate Nurse (LPN or RN) to join our team at Rogerson Communities. As a Resident Care Nurse, you will play a vital role in providing high-quality care to our residents, ensuring their physical and emotional well-being.
Key Responsibilities
• Evaluate, train, and supervise direct care staff in activities of daily living, hygiene, nutrition, and infection control to ensure quality care consistent with Assisted Living and dementia care guidelines.
• Document and monitor the medication management program and distribution of medications to residents by CNAs, providing competency training for all resident care assistants in demonstrating competence in Self-Administered Medication Management Program (SAMM).
• Train and supervise staff in medication administration and coordinate with pharmacist and other suppliers/vendors for medication delivery and accuracy, maintaining all medication sheets updated.
• Administer non-injectable medications such as orals, drops, inhalers, and topical ointments to residents whose service plans specify, maintaining written documentation.
• Document and monitor health records and medical information, dementia evaluation history, and current records reflecting residents' physical and mental health.
• Collaborate with the Lead Care Nurse in planning the monthly staff schedule and monitor and communicate medical issues, program changes, family concerns, and staff issues to all other shift managers and resident care team leaders.
• Implement quality control/monitoring procedures to ensure that resident care staff performs their routine duties during all shifts and coordinate emergency response plans and emergency services for residents, training staff on all shifts for appropriate response to emergencies and company policies.
• Monitor and assess health status of residents, triage emergency situations, and coordinate the delivery of medical or nursing care with certified home health agencies, nurse practitioners, physicians, and rehab services.
• Act as a liaison to families, physicians, and health agencies regarding changes and status in residents' health and medication issues and take an active role in care planning with the Lead Care Nurse, AL Program Director, Executive Director, other departments, outside service providers, and families, directing resident care staff to implement changes and achieve goals of the care plan.
• Assist the AL Program Director in monitoring and assuring the structure of activity programs, including dining and toileting routines, monitoring of staff and care, and participate actively in staff orientation, recruitment, initial training, and ongoing staff education programs to comply with Assisted Living regulations.
• Coordinate wellness activities for residents, including vital signs, weights, nutritional monitoring, and resident education programs regarding health issues, train staff in the usage of adaptive equipment needs of residents, and monitor use.
• Investigate resident and employee incidents, identify safety hazards, and report to the Lead Care Nurse and Executive Director to institute corrective action and share in the responsibility of maintaining an environment that is conducive to the safety, security, physical, and emotional well-being and cognitive support of residents.
• Maintain inventory of medical supplies, personal care, and incontinent needs, monitor, and report to the Office Manager any expenses charged back to families and work with the Lead Care Nurse and other departments to maintain the AL/Nursing operating budget.
