Hospice Social Worker Job Description

Anew Hospice is seeking a compassionate and skilled Social Worker to join our team in Fort Wayne. As a Social Worker, you will play a vital role in providing emotional and practical support to patients and their families during a difficult time.

Key Responsibilities:

  • Assess the psychosocial status of patients and families, and communicate findings to the interdisciplinary team.
  • Provide counseling and support to patients and families, and educate them on advanced directives.
  • Develop and implement individualized plans of care, and participate in interdisciplinary team meetings.
  • Collaborate with healthcare professionals to provide comprehensive care, and maintain accurate clinical records.
  • Provide information and referral services to patients and families, and serve as a liaison between patients and community agencies.

Requirements:

  • Master's degree in Social Work from a Council on Social Work Education accredited program.
  • Minimum one year of experience in healthcare, with hospice experience preferred.
  • Excellent communication and organizational skills, and current CPR certification.
