Job Summary:

We are looking for a dedicated and detail-oriented LPN to provide skilled nursing care to patients in their homes. The ideal candidate will have a strong background in medical-surgical experience and be able to work independently with minimal supervision.

Responsibilities:
  • Participate in age-specific care and interaction, ensuring that patients receive personalized attention and care.
  • Obtain daily schedules of patients to be seen and verify each appointment to ensure timely and efficient care.
  • Identify common recurrent patient problems, symptoms, and behavioral changes, and document these in the patient's nursing notes.
  • Instruct patients, families, and caregivers in the development of treatment plans, and document instructions in the patient's nursing notes.
  • Administer prescribed treatments as permitted by state law and document physician's orders and interventions received by the LPN.
  • Inform physicians and other personnel of changes in patient condition and needs, and document prompt physician contact and telephone orders implemented without error.
  • Provide patient, family, and caregiver education and instruction, and document these in the patient's nursing notes.
  • Submit daily patient visit and mileage logs, supplies charges, and notes whether patients were seen on the schedule.
  • Participate in orientation of Home Health Care staff members, and complete proper training in pertinent areas of the department to ensure good quality care of all patients.
  • Participate fully in the department's Performance Improvement Program, and provide input and evaluation related to any monitors completed.
  • Participate in Safety, Infection Control, Hazardous Communication Procedures, and CPR in services annually, and have on file current CPR card and proof of in-service participation.
  • Participate in patient coordination of services meetings, and update, revise, and document changes in patient clinical status as part of patient care under the direction of an RN.
  • Implement Infection Control and Safety measures per policy and procedures, and utilize sharps containers when indicated, wear gloves, and follow hand washing and bag technique according to policy.
Requirements:
  • Must possess sufficient dexterity to prepare medications, perform venipunctures, and give injections on a daily basis.
  • Must be able to operate and handle medical equipment and technologically complex devices, including but not limited to keyboards, monitors, and IVACs on a daily basis.
  • Must be able to write neatly and legibly on a daily basis.
  • Must be able to lift and carry adult and pediatric patients, as well as monitors, oxygen tanks, backboards, boxes of supplies, and other equipment on a daily basis.
  • Must be able to push and pull transfers and transports of patients and/or various equipment, including but not limited to beds, wheelchairs, patients, audio visual equipment, carriers, carts, and monitoring equipment.
  • Must be able to run in an emergency, climb stairs, bend, stoop, and carry equipment.
  • Must have a valid driver's license, automobile insurance, and a dependable vehicle.
  • Must possess a sense of touch to palpate pulses, veins, body temperature, and for assessment of possible physical abnormalities.
  • Must be able to hear patients speak, obtain history, and complete physical assessment, including but not limited to auscultation with a stethoscope.
  • Must be able to see to assess patient condition and perform various procedures, including but not limited to preparing and giving medications, evaluating cardiac, blood pressure, and pulse oximetry readings.
  • Must have color perception to evaluate skin color, circulation, abrasions, and body fluid color.
  • Must be able to successfully pass a medication administration test with a score of 80% or greater.
  • Must be able to count, add, subtract, multiply, and divide to record information such as patient intake and output.
  • Must be able to remember all activities related to the care of the patient.
  • Must have reading skills in the English language to comprehend all materials regarding all aspects of patient care within the limits of the essential performance standards identified in the appropriate job description.
  • Must be able to write in English neatly and legibly on a daily basis with a high degree of accuracy.
  • Must be able to speak clearly to verbally communicate (in English) all aspects of patient care, including but not limited to physicians, patients, family, and other healthcare workers.
  • Must be able to assimilate complex data concerning patient diagnosis and in all unit procedures.
  • Must be able to utilize inductive and deductive reasoning in the care of the patient.
