Job Summary:
Cerebral Palsy Association of the North Country is seeking a dedicated and compassionate Direct Support Professional to provide high-quality care and support to individuals with developmental disabilities in our Individual Residential Alternatives (IRAs) and community settings.
Key Responsibilities:
- Provide Direct Care: Ensure the health, safety, and well-being of individuals in our care, including providing personal care, nutrition, and domestic skills.
- Develop and Implement Person-Centered Plans: Work with individuals to create and implement personalized plans that meet their unique needs, preferences, and interests.
- Teach and Assist: Provide training and support to individuals in areas such as money management, adaptive and technological device use, and social, educational, and recreational activities.
- Follow Health Care Treatment Orders: Administer medication and teach fire and emergency safety procedures to individuals.
- Transportation: Safely transport individuals to appointments, activities, and other destinations, utilizing Agency vehicles or personal vehicles when necessary.
- Communication: Maintain regular contact and communication with individuals' families and significant others to ensure seamless support and care.
- Documentation: Accurately document all aspects of care, including cash ledgers and receipts for purchases.
- Flexibility: Demonstrate flexibility in scheduling, including working evenings, overnights, weekends, and holidays as needed.
Requirements:
- Education: High School Diploma or GED preferred.
- Training and Certification: Successful completion of Agency orientation and training program, Medication Certification training program, and NYS Driver's License with two years of driving experience.
- Physical Demands: Ability to lift and carry up to 25 lbs, lift and support individuals with and without assistance, and shovel snow.
- Other: Reliable personal vehicle for business use, ability to operate personal and Agency vehicles, and ability to walk and stand for extended periods.
