Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs, including OASIS assessments.
  • Develop and implement individualized treatment plans in consultation with physicians.
  • Collaborate with the interdisciplinary team to ensure seamless care delivery.
  • Teach, supervise, and counsel patients and their families on physical therapy programs.
  • Supervise Physical Therapist Assistants and communicate effectively with other healthcare professionals.
  • Document patient progress and communicate changes to physicians and other team members.

Requirements:

  • Graduate of an accredited Physical Therapy program.
  • Current licensure to practice as a Physical Therapist in the State of Florida.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and reliable transportation.
  • At least one year of experience as a Physical Therapist, with home healthcare experience preferred.
