About This Hospice Registered Nurse Position

HopeHealth is seeking a skilled Registered Nurse to join its affiliate HopeHealth Hospice & Palliative Care team. This full-time position will be part of our Home North A team, serving the areas north of Providence.

Key Responsibilities
  • Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
  • Implement individualized plans of care and recommend revisions as necessary.
  • Consult with patients, families, and caregivers to educate them about disease processes and teach self-care techniques.
  • Provide medication, diet, and other instructions as ordered by physicians.
  • Initiate preventive and rehabilitative nursing procedures.
  • Coordinate patient and family services with interdisciplinary team members to develop and revise patient plans of care.
  • Inform physicians and other personnel of changes in patients' needs and outcomes of interventions.
Requirements
  • RN required, BSN preferred.
  • 1-3 years of related experience.
  • Strong nursing assessment, clinical skills, and communication skills.
  • Home care and/or related experience preferred.
