About the Role

We are seeking a skilled Registered Nurse to join our team as a Case Manager in Home Health Services. As a key member of our interdisciplinary team, you will provide high-quality nursing care to patients in their homes, working closely with patients, families, and healthcare professionals to achieve optimal health outcomes.

Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans to meet the unique needs of each patient.
  • Provide skilled nursing care, including wound care, medication management, and education on health and wellness.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Lead and direct the inter-disciplinary team to achieve individual patient goals.
Requirements
  • Graduate of an accredited school of nursing.
  • Current license as a Registered Nurse in the State of Hawaii required.
  • BLS Certification.
  • At least two (2) years' experience in a hospital preferred.
  • Medicare Home Health Agency experience preferred.
