Job Summary

We are seeking a skilled Care Manager RN to join our team at Swedish Health Services. As a key member of our care management team, you will play a vital role in coordinating patient care and services across the continuum.

About the Role

The Care Manager RN will be responsible for:

  • Coordinating patient care and services across the continuum, including acute care, rehabilitation, and community-based settings.
  • Working collaboratively with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals to develop and implement individualized care plans.
  • Assessing patient needs and identifying opportunities for improvement in care delivery and resource utilization.
  • Developing and implementing initiatives to improve quality, continuity, and cost-effectiveness of care.
  • Fostering positive relationships with patients, families, and healthcare providers to ensure seamless transitions and optimal outcomes.
Requirements

To be considered for this role, you must have:

  • A Bachelor's Degree in Nursing from an accredited program.
  • A current Washington State Registered Nurse License.
  • At least 3 years of experience as a registered nurse in an acute care setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to analyze complex data and develop effective solutions.
