About the Role

We are seeking a highly motivated and compassionate Street Medicine Advocate to join our team at Healthcare in Action. This role will play a critical part in supporting individuals experiencing homelessness in the Garden Grove, CA area.

Job Summary

The Street Medicine Advocate will work closely with members who are at risk of or experiencing homelessness, providing personalized support and guidance to help them manage their health and well-being. Key responsibilities include:

  • Building Trust and Relationships: Establish rapport with members through empathetic communication, fostering trust and encouraging positive behavior changes.
  • Health Coaching and Navigation: Collaborate with members to create strategies that minimize risks associated with common health conditions and behaviors, connecting them to relevant resources and services.
  • Community Linkages and Advocacy: Facilitate access to local, county, and state services, ensuring members receive comprehensive support and care.
  • Medication Support and Follow-up: Deliver medications and other essential supplies to members as needed, while following up regularly to assess progress and address concerns.
  • Regular Outreach and Engagement: Conduct regular outreach to onsite patients at Nav Center and offsite members, providing medical support and addressing their needs.
  • Medical Appointment Accompaniment: Provide accompaniment and support to patients for medical, dental, and other appointments, ensuring smooth transitions and optimal care.
  • Care Coordination and Documentation: Collaborate with healthcare staff and HIA team members to manage patient care, maintaining accurate records and documentation.

Requirements and Qualifications

To excel in this role, you will need:

  • Education and Experience: G.E.D./High School diploma required, plus 3+ years of experience working directly with patients experiencing homelessness, or an equivalent combination of education and training.
  • Relevant Skills and Abilities: Ability to communicate effectively, build strong relationships, and navigate complex social systems; proficiency in operating a computer, including logging into multiple systems.
  • Personal Qualities: Compassion, empathy, and a commitment to improving the lives of vulnerable individuals.
