We seek a compassionate and experienced Case Manager RN PRN to join MountainView Hospital's team.
About this OpportunityThis role involves managing Rehabilitative care to achieve optimal outcomes, requiring effective coordination within the Rehabilitation team and collaboration with the Rehab Program Director and Facility Case Management Director on departmental functions.
- You will deliver program orientation to patients/families/caregivers, discussing the case manager's role, Rehab philosophy, continued stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities.
- You will coordinate patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status through Family Conferences and other interactions.
- You will implement the patient's treatment plan that prioritizes their strengths, abilities, needs, and preferences, facilitating involvement throughout the rehabilitation process.
- You will document findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments, conveying the patient's social, financial, or discharge needs and preferences.
- You will cultivate consistent, positive patient interactions that enhance unparalleled patient satisfaction.
- A degree in Nursing is required.
- BLS certification is essential.
- Experience in case management or clinical nursing is necessary.
- InterQual knowledge is beneficial.
- Case Management certification is desired.
