Job Summary:

As a Nurse Care Specialist with IntelliChoice Home Care, you will provide one-on-one nursing care in the home to critical, complex clients focusing on tracheostomy and ventilator patients of all ages. Our team is dedicated to delivering exceptional patient care and we are seeking a skilled and compassionate RN to join our team.

Responsibilities:

- Provide high-quality, patient-centered care in the home setting
- Collaborate with our interdisciplinary team to develop and implement individualized care plans
- Conduct thorough assessments and evaluations to identify patient needs and goals
- Develop and implement education plans to promote patient understanding and self-management

Requirements:

- Current RN license
- CPR certification (in-person training only)
- TB skin test (current within last 12 months)
