Position Overview: As a Clinical Supervisor, you will play a vital role in overseeing a team of healthcare professionals, ensuring the delivery of high-quality care to patients in their residences. Your leadership will be instrumental in coordinating and implementing individualized care plans.
Key Responsibilities:
- Supervise a diverse team of healthcare providers, including Registered Nurses, Physical Therapists, and Home Health Aides.
- Oversee the execution of physician-directed care plans, ensuring all activities align with available resources.
- Conduct regular assessments of client needs and adjust care plans as necessary.
- Evaluate the performance and skill levels of healthcare staff through ongoing reviews and feedback.
- Assist in the recruitment and training of new staff members, fostering a culture of continuous improvement.
- Facilitate in-service education programs to keep staff updated on healthcare practices and regulations.
- Support staff development through performance evaluations and recommendations for advancement.
- Maintain effective communication with team members to ensure alignment on patient care goals.
- Provide guidance on nursing standards and organizational policies to all levels of staff.
- Engage in regular utilization reviews to uphold the quality of care provided.
- Build and maintain relationships with referral sources and community resources to enhance service delivery.
Qualifications:
- Bachelor's degree or equivalent in a relevant field.
- Current Registered Nurse licensure.
- A minimum of six months of supervisory experience, along with at least one year in home care.
- Familiarity with Medicare home health agency regulations.
- Exceptional communication and leadership abilities, with a flexible approach to scheduling.
