Job Summary

This role involves providing comprehensive nursing care to clients in their homes, working closely with our multidisciplinary team to ensure seamless delivery of services. You will be responsible for assessing client needs, developing personalized care plans, and collaborating with healthcare professionals to achieve optimal outcomes.

Key Responsibilities:
  • Assessing Client Needs: Conduct thorough assessments of clients' physical, emotional, and social needs to identify areas for improvement and develop effective care plans.
  • Providing Comprehensive Nursing Care: Deliver high-quality nursing care to clients in their homes, including wound management, medication administration, and vital sign monitoring.
  • Collaborating with Healthcare Professionals: Work closely with doctors, therapists, and other healthcare professionals to ensure coordinated care and achieve optimal outcomes.
  • Documenting Progress: Accurately document client progress, including notes on treatment, interventions, and outcomes.
Requirements

To be successful in this role, you must have:

  • A current New Jersey nursing license in good standing.
  • Graduation from an accredited and approved nursing program.

Estimated salary range for this role is $65,000 - $90,000 per year, depending on experience and qualifications.
