Job Title: Obstetric Coordinator

Sea Mar Community Health Centers is seeking a highly skilled Obstetric Coordinator to join our team. As an Obstetric Coordinator, you will play a vital role in providing exceptional care to our patients and their families.

Job Summary:

The Obstetric Coordinator is responsible for coordinating prenatal care, tracking patient progress, and providing support to patients and their families throughout their pregnancy journey. This is a full-time position, Monday through Friday, 8:00 am to 5:00 pm.

Key Responsibilities:
  • Perform prenatal intake and obtain past medical history
  • Document patient information in the Electronic Health Record (EHR)
  • Provide updates and information to providers regarding shared OB and MSS clients
  • Assist patients with managing their pregnancy and linking them to social support services
  • Perform referrals, scheduling, and phone calls as assigned by the supervisor
  • Participate in weekly interdepartmental meetings, including the MSS meeting and the OB provider meeting
  • Manage the referral process, including tracking and ensuring patient attendance
  • Complete prenatal contracts and paperwork with clients and provide a brief orientation of services at their intake appointment
  • Send letters and make follow-up phone calls to patients for planned visits
  • Advocate for patient services with community, social service, and medical providers
  • Connect patients to Sea Mar and non-Sea Mar resources as appropriate and track all resources available to patients
  • Track patient adherence with the plan of care in the EHR and communicate outcomes to providers
  • Prepare monthly reports for provider meetings and monthly updated reports of the prenatal panel
  • Ensure the OB Database is updated regularly when knowledge of deliveries, transfer of care, miscarriages/terminations of pregnancy, changes to EDD, and changes of Provider
Requirements:
  • High school diploma and continued education in areas of health or social services, or at least one year of experience in those areas
  • 2 years' experience in a health and human services setting is preferred
  • Must be able to work independently, prioritize workload, and meet deadlines
  • Must have critical thinking skills and maintain confidentiality
  • Excellent organizational skills and ability to handle a variety of tasks simultaneously
  • Knowledge of medical terminology and/or behavioral health topics
  • Strong decision-making and prioritization skills
  • Ability to work respectfully and professionally with the community, patients, families, and staff
  • Able to work effectively in a multi-cultural environment with a diverse population
  • Sympathetic, mature, responsible, and reliable
  • Strong patient engagement, interpersonal, and communication skills and ability to establish a therapeutic relationship with the patient
  • Knowledge of the Patient-Centered Medical Home Model and motivational interviewing skills a plus
