Job Title: Physical Therapist

Blossom Ridge Home Health and Hospice is seeking a skilled Physical Therapist to join our team in Sacramento. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Job Summary:

As a Physical Therapist, you will be responsible for providing skilled therapy intervention and treatment to patients in a home setting. This includes developing and implementing treatment plans, assessing patient progress, and educating patients and caregivers on proper care and discharge procedures.

Responsibilities:
  • Provide skilled physical therapy in accordance with physician's plans of treatment.
  • Assist physicians in evaluating and assessing client's level of functioning.
  • Treat clients to relieve pain, develop and restore function, and maintain maximum performance.
  • Observe, record, and report client's reaction to treatment and any changes in condition.
  • Educate client/caregiver and healthcare team on physical therapy phases and use of prosthetic and orthotic devices.
  • Provide staff in-service education as directed by Director of Patient Care Services.
  • Assess and regularly reevaluate client needs, developing, implementing, and revising plans of care.
  • Initiate diagnostic, preventive, and rehabilitative procedures as appropriate to client's care and safety.
  • Assist with plan of care and maintain continuity of care for multidisciplinary clients.
  • Develop, prepare, and maintain individualized client care progress records with accuracy and timeliness.
  • Participate in agency's quality improvement program.
  • Keep abreast of physical therapy trends and knowledge for service provision, documentation, and care coordination.
  • Participate in staff meetings, in-service programs, and utilization reviews.
  • Supervise Physical Therapist Assistant at least every 30 days.
Requirements:

To be successful as a Physical Therapist at Blossom Ridge Home Health and Hospice, you must possess the following qualifications:

  • Licensed by the Physical Therapy Examining Committee of the California Board of Medical Quality Assurance.

Environmental Elements:

Incumbent works in an office environment sometimes with moderate noise levels, controlled temperature conditions, and no direct exposure to hazardous physical substances. Employees may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures. Incumbent will have to travel to a variety of patient homes and perform in conditions that vary greatly depending upon the client's home environment.
