Palliative Care Nurse

UPMC Family Hospice and Palliative Care is seeking a dedicated Registered Nurse to provide exceptional patient care in Fayette Green and Washington county areas.
This rewarding career offers direct one-on-one interaction with patients, making a positive impact in our communities by supporting individuals and their families on a collaborative team that works closely with physicians, social workers, and nursing staff to meet patient needs.
As a valued member of the team, you will work Monday through Friday during daylight shifts from 8:00 am to 4:00 pm in the Pittsburgh and surrounding areas. There may be rotations for on-call, holiday, and weekend coverage.

Your final title and compensation will be determined by your education, experience, and certifications.
ResponsibilitiesYou will be responsible for coordinating case management activities within an interdisciplinary healthcare team to ensure appropriate delivery of services to patients. You will also focus on customer service, complete initial and comprehensive assessments of patients, and take action to improve your knowledge, skills, and performance based on feedback or self-identified developmental needs.
You will initiate, coordinate, and ensure ongoing communication with the physician and other members of the healthcare team, including evaluating patients and providing family-centered health teaching. You will also provide direct patient care, evaluate outcomes, and adjust the nursing care process as indicated to ensure optimal patient care.
You must have a current license as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. You must also possess CPR certification based on AHA standards, which include both a didactic and skills demonstration component within 30 days of hire.
The estimated salary range for this position is between $60,000 and $80,000 per year, depending on education, experience, and certifications.
