Key Responsibilities- Provide direct nursing care to patients in their homes, focusing on quality of life and symptom management.
- Participate in the planning and coordination of patient care, working closely with our interdisciplinary team.
- Administer medications as prescribed, ensuring patient safety and adherence to treatment plans.
- Provide patient and caregiver education, empowering individuals to manage their care and make informed decisions.
- Offer emotional support and comfort to patients and their families, promoting dignity and respect throughout the care process.
- Assess patient needs and develop individualized care plans, collaborating with our RN Case Managers to ensure comprehensive support.
- Document patient information accurately and timely, adhering to established agency guidelines and nursing practice standards.
- Current Oregon LPN license in good standing.
- One year of hospice or home health nursing experience, with a strong preference for those with experience in palliative care.
- Excellent clinical skills, with the ability to apply the principles of nursing care in a hospice setting.
- Strong problem-solving and teaching skills, with the ability to communicate effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a focus on collaboration and mutual respect.
- Current CPR certification and a valid Oregon driver's license, with reliable transportation and proof of insurance.
