Job Title: LPN/RN Medication Coordinator

We are seeking a skilled MEDICATION LICENSED NURSE (RN/LPN) to join our team at The Reserve at Oswego in Oswego, Illinois.

About the Job:

  • Manage and control all phases of resident medications, treatment, and care as assigned.
  • Supervise the facility in the absence of the Administrator.
  • Implement corporate mission and administer policies and procedures to promote the desired business image of the community.

Responsibilities:

Primary Duties and Responsibilities:
  • Delegate authority, responsibility, and accountability necessary for carrying out assigned tasks.
  • Provide status reports to incoming nursing staff and obtain reports from outgoing nursing staff.
  • Chart and maintain medical charts and/or progress notes with pertinent resident information.
  • Complete Incident/Accident report for residents and/or personnel as needed.
  • Maintain accurate, complete, and confidential resident medication and care records according to established procedures.
  • Ensure safe and comfortable accommodations for residents and respond to inquiries for information or assistance in a prompt and courteous manner.
  • Communicate with doctors, families, and other appropriate parties concerning resident status, change in status, need for an appointment, and other related needs.
  • Develop and maintain an excellent working relationship with staff, inter-department personnel, and other departments and facilities within the community.
  • Create and maintain an excellent atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
  • Pass morning, afternoon, and evening medications, staying with each resident until the medication is consumed, and chart appropriately.
  • Assist in all care of residents as needed.
  • Receive, record, and/or restock all medications from the pharmacy.

Requirements:

  • Must have active and current nursing licensure (e.g., RN, LVN, LPN).
  • Minimum six months on-the-job experience as a licensed nurse required; experience providing care to seniors preferred.
  • Proficiency in English preferred.
  • Receives and maintains CPR certification.
  • A minimum of four years in clinical nursing practice or long-term care administration/supervision required.
  • Supervisory/administrative experience preferred.
  • Ability to easily lift and carry 50 lbs.

Estimated Salary Range:$65,000 - $85,000 per year depending on qualifications and experience.
