Nurse Care Coordinator

Mass General Brigham seeks a skilled RN Case Manager to facilitate patient care transitions, ensuring seamless and effective post-acute services. As a vital member of our healthcare team, you will coordinate with patients, families, healthcare providers, and insurers to create personalized discharge plans.
About the RoleWe are looking for an experienced Registered Nurse with a strong background in case management, acute care, and healthcare reimbursement. Your expertise will be instrumental in identifying high-risk patients and developing strategies to prevent readmissions. In collaboration with our multidisciplinary team, you will assess patients' needs, identify resources, and ensure timely and accurate insurance reviews.
Key Responsibilities:- Develop and implement comprehensive discharge plans for patients requiring post-acute care or follow-up services.
- Collaborate with healthcare teams, patients, and families to address complex medical and social needs.
- Utilize nationally accepted criteria to determine medical necessity and appropriate level of care for hospitalized patients.
- Routinely review patients' health records and communicate with healthcare teams to ensure continuity of care and compliance with regulations.
- Complete timely and accurate insurance reviews as requested by third-party payers.
To succeed in this role, you must possess:
- A Graduate degree from an accredited school of nursing, BSN preferred.
- Current licensure as a Registered Nurse in the Commonwealth of Massachusetts, with a minimum of three years' clinical experience.
- Evidence of continued education and professional development, CCM or ACMA certification preferred.
- Prior experience in general acute care hospital settings, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery.
- Knowledge of infant, child, adolescent, adult, and older adult patient special needs and behaviors, preferred.
