Job Summary:

We are seeking a skilled Caregiver to join our team, providing on-site direct behavioral care services to elderly, mentally disabled, chronically ill, or physically challenged consumers.

Key Responsibilities:

  • Assist with daily living skills, including personal care and hygiene
  • Provide client care needs, including medication monitoring and transportation services
  • Manage client cases, ensuring effective communication and documentation
  • Offer supportive counseling and crisis management services
  • Plan and facilitate recreational and social activities
  • Communicate significant changes in clients' behaviors and notable incidents to team leaders
  • Document client charts and advocate for community resources
  • Ensure a safe and therapeutic environment, adhering to safety standards, government regulations, and corporate policies

Requirements:

  • Bachelor's degree in mental health or a related field, or a High School Diploma/GED with 4 years of related work experience
  • Valid driver's license and a clean driving record
  • Ability to pass a physical, drug screen, and background check
  • Must be at least 21 years old
