$45,000 - $60,000 per year

Job Description:

As a Hospice Home Health Care Specialist, you will work under the supervision of a registered nurse to provide various services for patients in their homes. Your primary responsibilities include observing patients, reporting observations, and documenting care performed.

You will be assigned to patients in a manner that promotes quality, continuity, and safety of their care. Your duties may include performing personal care needs, meeting safety needs, giving personal care, assisting with dressing and undressing, planning and preparing nutritious meals, and taking and recording vital signs.

Additional responsibilities may include providing proper care and observation of patient's skin, reporting on patient's condition, and participating in quality assessment performance improvement teams.

Required Skills and Qualifications:
  • Experience working in home healthcare or a related field
  • Ability to perform physical tasks required for the job, including lifting and transferring patients
  • Excellent communication and interpersonal skills
