VNA & Hospice of the Southwest Region Inc, a non-profit home health and hospice care agency in Vermont, is seeking a skilled Physical Therapist to join our team. This full-time position offers a competitive salary and a range of benefits, including a $10,000 sign-on bonus.

Job Overview

This Physical Therapy role involves evaluating, developing, implementing, and documenting treatment plans for patients of all ages. As a Care Manager and/or team member, you will coordinate patient care with other clinical team members and supervise and direct the activities of PTAs, PT students, and home care aides.

Key Responsibilities:
  • Evaluate and assess patient needs, developing personalized treatment plans to promote optimal functional independence and safety.
  • Provide skilled physical therapy treatment, including therapeutic techniques, patient/caregiver education, and assessing patient status and progress on each visit.
  • Act as a case manager, coordinating patient care with other clinical team members and supervising the activities of PTAs, PT students, and home care aides.
Requirements:
  • Bachelor's degree in Physical Therapy from an accredited university or college required; Master's degree preferred.
  • At least 2 years of experience in Physical Therapy, preferably in home health settings.
  • A valid State Driver's License is required.
