Job Summary: We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Patient Support Unit in San Antonio, TX. As a member of our team, you will play a vital role in providing comprehensive, coordinated care to our Medicare-aged patients.
Key Responsibilities:
- See patients in various clinics, including home, telemedicine, and mobile visits
- Collaborate with our employed, contracted, and affiliated physicians, advance practice clinicians, and clinic staff to provide high-quality care
- Educate providers on proper coding, risk adjustment metrics, and quality protocols
- Interview patients to ensure proper documentation and preventative screenings
- Track performance using metrics related to our organization's strategic direction and operational needs

Requirements:
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Current licensure through the Texas Board of Nurse Examiners or licensed physician assistant by the Texas State Medical Board
- Valid DEA license or ability to obtain one prior to employment
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant
- Comfortable managing a primarily Medicare-aged patient population
Preferred Qualifications:
- Familiarity with primary care and chronic condition codes
- Bilingual (English/Spanish) fluency
- Working understanding and support of a managed care environment
