Responsibilities:

  • Assess and evaluate patient needs, identifying mutually agreed-upon goals for patient care plans
  • Provide speech therapy services, including assessment, evaluation, procedures, teaching, and training activities as outlined in patient care plans
  • Report patient status to Clinical Supervisor and referring Physician
  • Document daily changes and updates to patient care plans, including discharge planning
  • Attend staff meetings, team conferences, and educational in-services per agency requirements
  • Perform all OASIS time point assessments per Medicare Criteria and submit recertification paperwork per policy and procedure

Qualifications:

  • A current unencumbered State Professional License
  • Current driver's license and proof of insurance
  • One-year experience as a home care, professional Speech Language Pathologist, and is competent in performing comprehensive assessment and OASIS-C
  • The ability to make sound professional clinical judgment
  • The ability to assess and document patient needs and formulate individualized patient care plans to meet those needs
  • Proficiency in clinical skills
  • Proficiency in Microsoft Office
