Geriatric Nurse Practitioner

Estimated Salary: $48,000 - $68,000 per year based on location and experience.

This Geriatric Nurse Practitioner role requires a skilled healthcare professional who can provide high-end medical service, compassionate care and industry leading solutions to our most vulnerable patient populations. Main Responsibilities:

• Establish and maintain effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
• Conduct initial comprehensive assessments on new patients and provide ongoing care, including visits to address changes in condition
• Develop patient management care plans and update them as needed when changes in condition warrant or following hospitalization
• Perform post discharge visits to ensure patient stability is consistent with their goals of care
• Participate in HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery

Requirements:

• Certification through the American Academy of Nurse Practitioners (AANP), or American Nurses Credentialing Center (ANCC)
• Current, unrestricted NP licensure in State of Practice
• Nurse Practitioner must actively hold, unrestricted RN licensure in State of Practice
• Access to reliable transportation; if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy
