Pinnacle Home Care Inc. is seeking a skilled Speech Therapist to join our team of dedicated healthcare professionals. As a Speech Therapist, you will play a vital role in providing high-quality care to our patients in their homes.
Key Responsibilities:
- Evaluate patients' communication and swallowing abilities, and develop personalized treatment plans to enhance their quality of life.
- Conduct comprehensive assessments, including Outcome and Assessment Information Set (OASIS) evaluations, to identify patients' needs and goals.
- Collaborate with healthcare teams to develop and implement interdisciplinary care plans that address patients' communication and swallowing needs.
- Provide education and training to patients, families, and caregivers on effective communication strategies and techniques.
- Document patients' progress, treatment plans, and outcomes in a timely and accurate manner.
Requirements:
- Graduate of an accredited Speech Therapy program or equivalent education.
- Licensed in the State of Florida as a Speech Therapist.
- Two years of clinical experience as a Speech Therapist.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
