License: Must have license in hand

Certifications: CPR, BLS

Job Responsibilities:

1. Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures.

2. Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.

3. Conduct home visits and attend periodic office meetings.

4. Manage a caseload of 5-10 patients per day, equating to roughly 6-8 points.

5. Perform various types of cases, including Start of Care, Wound Care, and Follow-Up Visits.

6. Open to on-call and weekend rotations as required by staffing.

7. Cover assigned territory within a specified mileage radius.

8. Maintain productivity of 30 units minimum weekly.

9. Adhere to the dress code of wearing scrubs.

Requirements:

- Proficiency in HomeCare HomeBase (HCHB) and experience within the last 6 months or completed retraining.

- Proficiency in OASIS, used in the last 6 months, or completed retraining.

- Reliable transportation, valid driver's license, and agency-required liability insurance.

- Willingness to travel up to 75 miles from the assigned care center.

- Ability to complete all documentation within 24 hours of patient appointments.

- Acceptance of every patient on the schedule regardless of distance.
