Job Summary

We are seeking a highly skilled and experienced Nurse Practitioner or Physician Assistant to join our team in Seattle. As a key member of our post-acute care program, you will be responsible for providing high-quality primary care to patients in a skilled nursing facility setting.

Key Responsibilities:
  • Perform primary care in a skilled nursing facility setting for short-term and long-term patients
  • Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
  • Order and interpret lab results
  • Prescribe and monitor medications
  • Consult with physicians and coordinate patient care with other healthcare providers
  • Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
Requirements:
  • 1 or more years of experience in primary care
  • Post-acute care experience a plus but not required
  • Active and unrestricted nurse practitioner license in the state of Washington
  • Active and unrestricted DEA for the state of Washington
