We are seeking a highly skilled and compassionate Full-Time Home Health Registered Nurse Case Manager RN to join our team at Signature Healthcare at Home. As a key member of our team, you will be responsible for providing high-quality patient care and case management services to our patients in their homes.
Responsibilities:
- Conduct comprehensive assessments of patients and their families to identify their needs and develop individualized care plans.
- Provide skilled nursing care and interventions to patients in their homes, including wound care, medication management, and education.
- Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Manage patient caseloads, prioritize tasks, and maintain accurate records and documentation.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
- Graduate of an accredited school of nursing with a current RN license in the state of practice.
- 1-2 years of recent acute care experience in an institutional setting, with 1-2 years of recent experience in home health preferred.
- Excellent communication, problem-solving, and organizational skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
