Job Title: Lead Registered Nurse

Job Summary:

The Lead Registered Nurse will serve as an integral member of our nursing and health center leadership teams. This role will assist and support the CCI team in providing high-quality, direct, evidence-based patient care and care management for high-risk patients with chronic illnesses.

Key Responsibilities:
  • Develop and maintain a working rapport with patients to ensure optimal care and outcomes.
  • Lead and participate in clinical huddles, pre-visit planning, and identifying complex care patients.
  • Conduct patient appointments, collecting health history, assessing, screening, triaging, analyzing, and identifying patient healthcare needs and care gaps.
  • Provide medical advice and discharge instructions to patients, utilizing evidence-based resources for pediatric, adult, and obstetric populations.
  • Review and reconcile medical and vaccine records, completing required forms and paperwork.
  • Reconcile medication history and counsel patients on medication use, benefits, risks, and potential side effects.
  • Coordinate clinical care, transitions of care, and support the referral process.
  • Counsel and educate patients on chronic disease management, primary prevention activities, abnormal labs, and diagnostic imaging.
  • Execute treatment plans as directed by the Director of Nursing and/or clinician, evaluating results and providing point-of-care testing, vaccination, and medication administration.
  • Provide verbal and written information in the patient's preferred language, utilizing in-person or telephonic interpreters.
  • Maintain a panel of care management patients, developing care plans, setting patient-centered goals, and assessing patient self-advocacy, self-management, and barriers to care.
  • Collaborate with population health and lead monthly care management huddles, coordinating patient care with community health workers.
Requirements:
  • Active RN license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact.
  • Minimum of three (3) years of nursing experience, with progressive leadership experience a plus.
  • Current BLS/CPR certification required.
  • Experience with documenting in an Electronic Medical Record (EMR) – specific knowledge of eClinicalWorks is a plus.
  • Prior experience in a lead or supervisory capacity a plus.
  • Written and oral fluency in English.
  • Spanish speaking is a plus.
