Key Responsibilities
  • Conduct thorough physical health assessments for clients, including HIV risk assessments, general first aid, and triage activities for sick calls.
  • Routine screening for substance abuse, developmental health, mental health, and physical body checks for signs of abuse.
  • Manage and train clients regarding chronic conditions, such as asthma and diabetes.
  • Take vital signs, heights, and weights, and document observations and activities.
  • Provide immunizations and document same.
  • Provide first aid and emergency care as needed.
  • Execute medical orders, including tests, tasks, and hospitalizations, as ordered by a physician or nurse practitioner.
  • Obtain lab specimens and administer hearing and vision tests.
  • Train and educate clients on sexual reproductive health.
  • Administer medications and manage medication reconciliation.
  • Ensure required documentation for all case actions is completed within established timeframes.
  • Maintain professional boundaries with clients and staff.
  • Maintain a safe and secure medical environment for clients.
  • Accept medication deliveries, check for accuracy, and store safely.
Requirements
  • Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in New York State.
  • Current CPR certification.
  • Completion of required courses in Child Abuse and Infection Control.
  • Valid Driver's License with a satisfactory driving record.
